 Please order your repeat prescriptions 10 days in advance of needing medication and allow 7 days between ordering and collecting it at your community pharmacy.

New Patient Registration

appointment_1If you wish to register with the practice we will require some information.  If possible, please bring your Registration Card (Form GP4) if you have one.  You will be asked to complete the details and sign the form.  If your card is not available you will be asked to complete a registration request form.

Alternatively, you may complete a patient registration form online, print it off and bring with you when you come to register at the practice.

All new patients are also asked to complete a medical questionnaire giving us some information about your past medical history.  This is because it can take sometimes take a considerable time for us to receive your medical records.   If you take regular medicines you will also be asked to make a registration appointment to meet the GP although any patient new to the practice can request an appointment with the doctor.

Your registration will be with the practice rather than a specific doctor however you may request to see any doctor within the practice.  If you express a preference we will record that preference in your medical records. 

New Patients

Due to concerns about our capacity to provide safe patient care we are currently unable to accept applications to register at this practice if you are already registered with a GP in Arbroath or Friockheim.
